Online Registration
To prepare for your visit to the DALLAS ENDOSCOPY CENTER LLC, your medical history and insurance information is needed. You can expedite this process in several ways. You can download the information sheets, print them and fill them out; have them ready for your visit. Another option is to fill them up on line. If for any reason your computer does not support this functionality contact us and let us help you. This needs to be done in advance, by telephone. During the telephone registration, you will speak with a Patient Representative or a Registered Nurse. Important instructions will be given to you regarding your procedure.
Telephone registration may take approximately 15 minutes to complete. All information is confidential. If you have not been contacted within 48 hours prior to your procedure date, please call us at 214.520.8235. |
