To prepare for your visit to the DALLAS ENDOSCOPY CENTER LLC, your medical history and insurance information is needed. Help us expedite this process by registering in advance. Download the information sheets, fill them out and then print them; have them ready for your visit. You can also fill them up on line and submit via e-mail through our secure site. For pre registration by telephone a Patient Representative or a Registered Nurse will conduct the interview. All information is confidential. Important instructions will be given to you regarding your procedure. This process may take approximately 15 minutes to complete.
